Services
Community Companion Home Care, LLC strives to provide excellent care and person centered services for participants in Western North Carolina. We go above and beyond to fight for what is best for our participants. We accept referrals from the Local Management Entity, guardians, family members and case managers. Residential
Residential Supports are provided to individuals who live in a community residential setting that meets home and community based characteristics.
These services are aimed at assisting the participant to acquire, improve, and retain skills in self-help, general household management and meal preparation, personal finance management, socialization and other adaptive areas. Assistance in activities of daily living when the participant is dependent on others to ensure health and safety.
Our AFLs (Alternative Family Living Homes) have to pass various health and safety requirements, state and local inspections and are trained to handle various situations from behavioral, medical and emergency situations.
Day Supports
We are proud of our day program Possibilities located at 81 South Main Street in Marion. Our day program is a massive 15,000 square foot. Our participants enjoy various activities including but not limited to: Zumba, Ice Cream Outings, Basketball, Bowling, Arts and Crafts, Visiting Local Establishments and much, much more. Day Supports provides assistance to the participant with acquistion, retention or improvement in self-help, socialization and adaptive skills. Day Supports focus on enabling the participant to attain or maintain his or her maximum functional level and is coordinated with any physical, occupational or speech therapies listed in the individual support plan.
Habilitative
We provide Community Living Supports and Community Networking Services.
Community Living Supports provides habilitation and skill building to enable the participant to acquire and maintain skills, which support more independence. It augments the family and natural supports of the participant and consists of an array of services that are required to maintain and assist the participant to live in community settings. It is individualized, specific, and consistent with the participant's assessed disability specific needs.
Community Networking provides individualized day activities that support the participant's definition of a meaningful day in an integrated community setting with persons who are not disabled. These services are designed to promote maximum participation in community life while developing natural supports within integrated settings.
Supported Employment
Supported Employment services provide assistance with choosing, acquiring, and maintaining a job for whom competitive employment has not been achieved. The intent of supported employment is to assist individuals with developing skills to seek, obtain and maintain employment or develop and operate a micro-enterprise. The employment positions are found based on individual preferences, strengths, and experience.
Initial supported employment includes pre-job training/education and development activities to prepare the participant for meaningful work-related activities. This may include job searching, career counseling, job shadowing, job interview skills, study skills and assistance in learning skills necessary for job retention.
Respite Care
Respite services provide periodic support and relief to the primary caregiver(s) from the responsibility and stress for the participant. It enables the primary caregiver to meet or participate in planned or emergency events, and to have planned time for him/her and/or family members.
Supported Living
Supported living provides a flexible partnership that enables a person to live in their own home with support that provides individualized assistance in a home that is under the control and responsibility of the participant. The service includes direct assistance as needed with activities of daily living, household chores essential to the health and safety of the individual, budget management, attending appointments, interpersonal and social skills building to enable the individual to live in a home in the community.
Persons living in a supported living arrangement shall choose who lives with him/her, are involved in the selection of direct care staff, and participate in the development of roles and responsibilities of staff.
